I really appreciate the reduction in silos. Psychiatry as advocacy

I hope that people are understanding… as the world diversifies and becomes more global… that all of these things interdigitate with one another, and that you really can't unpack them unless you understand them… If you don't understand a person's context, you cannot possibly understand their disease… Have you walked down that neighborhood? There are no grocery stores, there actually are no green spaces, there actually are no sidewalks. So that is intrinsically related to people's health. There is no broadband, there is a lot of noise, there’s toxic fumes in the air, these buildings are old… And to not understand how all of that impacts the child's health is to negate what it is to be a physician. Our biopsychosocial formulation is supposed to get at that. But I don't think that we do it in a way that is nuanced enough to understand how social determinants of health are based on these structural systems
